This Form is part of a pilot program and at present can only be used in the following locations:
- All NYC trial courts (Bronx, Kings, New York, Queens, and Richmond)
- Monroe County (all courts, except Town and Village Courts)
- Nassau County (all courts, except Town and Village Courts)
- Suffolk County (all courts, except Town and Village Courts)

This Form is not intended to be used to make blanket requests for remote appearances by phone or video. Attorneys wishing to request remote appearances as an accommodation -- either in more than one case before the same judge, or in cases before more than one judge -- are advised to use other means to contact the judge or judges before whom their cases are pending. Please do not submit this form to request blanket accommodations.
To the extent possible, we will keep your request - and any information submitted in support - confidential. All material will be reviewed by court personnel only as needed to determine whether your request can be granted.
